Thank you for applying to Youth With A Mission Turner Valley, Alberta, Canada. The following forms must be completely submitted in order for your application to be processed.

Please read the following instructions carefully.

Step 1: Fill out your "Student Application form" and submit when done. (Note: you cannot save the form and return later to finish, so please be aware that you will need to finish the main part of the application at one time.)

Step 2: Confidential Health form. Download the "Confidential Health Form" after you have completed the online version of your "Student Application form". Make sure to have your doctor or physician fill this out and return to us by mail or fax (Fax#: 1-403-933-3754).

Step 3: Reference Forms. To complete this section, you may either download and print the forms to give to your references, or you may give them the link below to complete the references online.

Remember to make sure you have a reference from both of the following:

- Teacher/Employer
- Pastor/Elder

Step 4: Application fee. Please forward the nonrefundable application fee of $25 (Canadian) with each application submitted. Submission of this fee helps us with the processing of your application.

Important: All applicants are encouraged to apply early; generally no later thatn six weeks prior to the start of the school. International applicants may require two months to process a visa for entrance into Canada.
